Back or lower back pain
Most back pain comes from the muscles and joints of the spine and settles within a few weeks. But if it spreads down your leg, your toes go numb or it won't go away, see a spine specialist, who will find the cause and choose treatment, usually without surgery.

Possible causes
These are the most common causes. Your doctor will name the actual cause after an examination and scans; it can't be pinned down from a description online.
Disc herniation or bulge
Pain spreads to the buttock and leg, sometimes with numbness or tingling in the foot.
Wear of the spinal discs and joints
A dull ache and stiffness, worse after long standing or sitting.
Spinal stenosis
Legs ache and tire when walking and feel better when you sit or lean forward.
Scoliosis and posture problems
Uneven shoulders or shoulder blades, a back that tires quickly.
Vertebral compression fracture
Sudden pain after a fall, or even without injury in older people with osteoporosis.
Book an appointment if
- the pain hasn't eased after 4–6 weeks
- the pain spreads to a leg or arm
- your fingers or toes are numb or tingle
- the pain keeps coming back
What you can do before your visit
- Stay gently active: short walks help more than bed rest.
- Avoid lifting and sudden bending or twisting.
- If the pain spreads down your leg, don't have manipulation or massage before you've been examined.
Take painkillers only as the leaflet says and if they're safe for you. Don't heat a fresh injury, and don't have joint injections without a doctor's examination.

Common questions
Do I need an MRI for back pain?
Not always. Without warning signs, an MRI usually isn't needed in the first weeks. The spine specialist will order one if the pain spreads down your leg and doesn't settle, if there's weakness or numbness, or if surgery is being considered.
Does a herniated disc always need surgery?
No. For most people the pain from a herniated disc eases without surgery. Surgery is offered for worsening weakness, bladder or bowel problems, or when long non-surgical treatment hasn't helped.
